If you would like to consider moving your company’s group medical insurance plan, please read the following:
This page sets out the medical declaration for the recommended health insurer. As the group secretary for the plan, you will be required to answer these questions to the best of your knowledge. Should you answer “yes” to any of these questions, we will require full details of treatment and will refer the information to the insurer’s underwriters for their consideration.
1 employee:
I declare to the best of my knowledge, no applicants to be covered:
- Have any day-patient or in-patient treatment or diagnostic tests* planned or anticipated
- Have had consultations or treatment for any heart or cancer conditions in the past three years
- Have had consultations or treatment for any joint disorder or disease (including arthritis, knee, shoulder and back problems), or any mental health, psychiatric or behavioural disorders in the last twelve months
2 to 9 employees (applying for CPME or MHD terms):
I declare to the best of my knowledge, no applicants to be covered:
- Have any day-patient or in-patient treatment or diagnostic tests* planned or anticipated
- Have had consultations or treatment for any heart or cancer conditions in the past three years
- Have had consultations or treatment for any mental health, psychiatric or behavioural disorders in the last twelve months
*By day-patient, Vitality means an admission to hospital or day-patient unit that required a period of supervised recovery but did not require an overnight stay. By in-patient, Vitality means an admission to hospital that required you to stay overnight. By diagnostic tests, Vitality means any test, x-ray or scan that is intended to diagnose an illness or injury, including participation in an enhanced screening or surveillance programme that is not part of the regular NHS population screening programme.
10 to 99 employees (applying for CPME terms), or 10 to 49 employees (applying for MHD terms):
I declare to the best of my knowledge, no applicants to be covered:
- Have had consultations or treatment* for any heart or cancer conditions in the last two years
- Have had consultations or treatment for any mental health, psychiatric or behavioural disorders in the last twelve months.
50 to 99 employees and you are applying for MHD underwriting terms:
- I declare that to the best of my knowledge no applicants to be covered have any ongoing heart or cancer conditions or related claims.
If you become aware of any changes to the information provided prior to the start date of the policy, you must notify us as this may affect the terms offered by the insurer.
