CPME medical declaration
If you would like to consider moving your company’s group medical insurance plan, please read the following:
This page sets out the medical declaration for the recommended health insurer. As the group secretary for the plan you will be required to answer these questions. Should you answer “yes” to any of these questions, we will require full details of treatment and will refer the information to the insurer’s underwriters for their consideration.
2 employees (applying for CPME terms):
- Do you or any applicant have any consultations, investigations or treatment planned or pending in the next 12 months (NHS or Private)?
- Have you or any applicant had any consultations, investigations or treatment in the last 12 months (NHS or Private)?
- Have you or any applicant ever been treated for, diagnosed with, or advised that they have a heart condition, cancer or mental illness?
3 - 9 employees (applying for CPME terms):
To the best of your knowledge, in the last 3 years have any of the employees or their dependants to be included had:
- Any treatment or consultations in relation to any type of cancer?
- Any type of heart or circulatory conditions?
- Any type of psychiatric or mental illness?
- Any operation of a major joint including hip, shoulder, knee, neck or back?
- Been advised in the next 12 months they will need, or may need an operation, treatment, test or investigations as an in-patient or day-patient?
10+ employees (applying for CPME terms):
Same as above for 3 – 9 employees but delete question 4.
If you become aware of any changes to the information provided prior to the start date of the policy, you must notify us as this may affect the terms offered by the insurer.
Member eligibility
This page sets out the group member eligibility rules for this insurer:
Freedom’s company policies are intended to provide cover for employees, directors and other designated members of an actively trading business, based in the UK, this is known as a group scheme.
The group scheme can cover:
- The member who must be either: A director, business partner, proprietor or PAYE employee of the policyholder; or – any other person the policyholder has allowed to be included in the group scheme as a member with our permission.
- The member’s partner (husband, wife, civil partner or partner who permanently lives at the same address as the member).
- The member’s or the member’s partner’s children (including adopted children) who normally live at the same address as the member.
Freedom’s definition of an eligible employee is someone who is employed by the company on a PAYE basis, a partner or equity partner or a registered director.
All members must be permanently resident in the UK.
Please speak to your consultant if you would like to provide cover for anyone that falls outside of this definition as we may need to seek authorisation from the insurer.
